Coach/Manager Registration

1. When prompted to select a participant to register, select yourself OR choose + NEW PARTICIPANT and add your name and information for this registration.
2. You will need your USA Hockey Card # and Level # to complete this registration.
3. You will need your AAU membership number to complete this registration.

The registration process will collect the following:

1. Personal Name and Contact Information
2. Volunteer Type (Coach, Team Manager, etc.)
3. USA Hockey Information
4. AAU Membership Information
5. Consent to Treat Form
6. Waiver of Liability Form
